Obat Tradisional

Obat TradisionalMenurut Undang-Undang Kesehatan No. 23 tahun 1992, yang dimaksud obat tradisional adalah bahan atau ramuan bahan yang berupa bahan tumbuhan, bahan hewan, bahan mineral, sediaan sarian (galenik) atau campuran dari bahan tersebut yang secara turun-temurun telah digunakan untuk pengobatan berdasarkan pengalaman.

Akhir-akhir ini dunia, khususnya dunia Barat mulai memalingkan kembali perhatiannya ke alam, yang terkenal dengan semboyannya back to nature, mengikuti jejak dunia Timur, khususnya Asia yang sampai detik inipun masih tetap memanfaatkan obat-obat dalam dalam upaya-upaya pelayanan kesehatan di samping obat-obat farmasetik. Kembalinya perhatian dunia Barat ke obat-obat alam ini tidak lain adalah karena kembali tumbuhnya kepercayaan masyarakat Barat bahwa obat-obat alamiah, termasuk obat-obat nabati, dapat memberikan peranannya dalam upaya pemeliharaan, peningkatan dan pemulihan kesehatan serta pengobatan penyakit. Di samping itu diyakini pula bahwa obat-obat alamiah kurang memberikan efek samping jika dibandingkan dengan obat-obat farmasetik. Obat-obat alam, termasuk obat-obat nabati diakui masyarakat mempunyai peranan dalam upaya-upaya pemeliharaan, peningkatan dan pemulihan kesehatan maupun pengobatan penyakit didasarkan atas pertimbangan, bahwa menurut pandangan Sistem Pengobatan Tradisional, obat-obat alam, termasuk obat-obat nabati, dapat mèmpengaruhi mekanisme pertahanan alamiah tubuh. Mekanisme pertahanan alamiah tubuh itu meliputi reaksi-reaksi spesifik maupun reaksi non spesifik yang berperan dalam proses eliminasi penyebab penyakit, khususnya mikroba.
Obat tradisional memiliki beberapa sifat yang perlu diperhatikan, yaitu : memiliki efek samping relatif kecil jika digunakan secara tepat, memiliki efek relatif lambat tetapi jelas manfaatnya, contohnya jamu pegel linu diminum pada malam hari, maka khasiatnya baru terasa pagi harinya, jamu pelangsing baru kelihatan efeknya setelah satu bulan mengkonsumsi secara teratur. Sifat lain yang perlu diperhatikan yaitu : bersifat holistik atau memiliki kombinasi efek dalam satu ramuan, lebih sesuai untuk penyakit metabolik (diabetes, hiperkolesterol, asam urat, hepatitis) dan degeneratif (rematik, hipertensi, maag, kanker, lemah syahwat, pikun), dan banyak yang bersifat promotif dan preventif (mencegah terjadinya penyakit).
Penggunaan obat tradisional juga dapat menimbulkan efek yang merugikan apabila tidak tepat dalam pemilihan bahan yang digunakan, dosis penggunaan, waktu penggunaan, cara penggunaan, susunan ramuan yang digunakan, dan ketidaksesuaian antara ramuan dengan tujuan penggunaan. Kebenaran dalam pemilihan bahan sangat mempengaruhi efek pengobatan yang diinginkan, contohnya jika diinginkan efek sebagai pelagsing, maka bahan tanaman obat yang bisa digunakan adalah lempuyang wangi, jangan sampai keliru dengan lempuyang emprit atau lempuyang gajah, karena memiliki efek yang berlawanan dengan lempuyang wangi, yaitu menambah nafsu makan. Ketepatan dalam dosis penggunaan juga perlu diperhatikan untuk menghindari timbulnya efek samping, contohnya daun seledri dapat digunakan untuk menurunkan tekanan darah tinggi, namun apabila digunakan lebih dari 400 gram dapat menyababkan pingsan. Contoh lain penggunaan telur-madu-susu untuk menjaga stamina, apabila dikonsumsi secara berlebihan dapat memicu penyakit diabetes. Waktu penggunaan obat tradisional juga harus diperhatikan. Jamu cabe puyang sebaiknya diminum pada masa awal kehamilan supaya memperkuat otot uterus, sehingga mengurangi resiko keguguran, tetapi bila diminum secara rutin pada masa kehamilan akan berakibat kesulitan dalam persalinan. Contoh lain penggunaan jamu kunir asem, jika diminum pada masa kehamilan dapat mengakibatkan keguguran. Ketepatan cara penggunaan pada daun kecubung yang memiliki khasiat sebagai antiasma, seharusnya digunakan dengan cara daun dikeringkan dan diserbuk untuk bumbu rokok, kemudian dihisap, jika digunakan dengan cara diseduh dan diminum akan mengakibatkan keracunan dengan gejala mata membelalak (midriasis).
Menurut SK Kepala Badan POM RI No. HK.00.05.4.2411, obat tradisional digolongkan menjadi tiga jenis, yaitu : jamu, contohnya : laxing, kunyit asam, tolak angin, dll; obat herbal terstandar, contohnya : lelap, diapet, kiranti, dll; dan fitofarmaka, contohnya : stimuno, tensigard, dll.

Tentang Obat

Tentang Obat

Definisi obat adalah senyawa atau campuran senyawa yang digunakan untuk diagnosa pengobatan, mencegah penyakit, mengurangi gejala, menghilangkan gejala, atau menyembuhkan penyakit. Sebagai contoh obat yang digunakan untuk diagnosa pengobatan yaitu barium sulfat yang digunakan sebagai zat kontras untuk roentgen saluran cerna, biasanya digunakan untuk mendiagnosa adanya usus buntu. Obat yang digunakan untuk mencegah penyakit, misalnya vaksin BCG untuk perlindungan terhadap tuberculosis, diberikan pada bayi yang baru lahir dengan tingkat bahaya ditularinya sangat tinggi. Obat yang digunakan untuk mengurangi gajala seperti obat-obat analgetik yang digunakan untuk mengurangi nyeri, contohnya parasetamol, aspirin, asam mefenamat, dll. Obat yang digunakan untuk menghilangkan gejala, misalnya obat batuk yang termasuk antitusif yaitu dekstrometorfan, gliseril guaiakolat. Obat untuk menyembuhkan penyakit yaitu obat-obat yang termasuk dalam golongan antibiotic, contohnya amoksisilin, eritromisin, metronidazol, dll.

Untuk mendapatkan khasiat yang diinginkan ada beberapa persyaratan yang harus dipenuhi oleh suatu obat yaitu : pertama, dosis obat harus tepat. Dosis yang tepat akan dapat membedakan suatu racun dari obat. Contohnya parasetamol pada dosis 500 mg 3-4 x sehari dapat digunakan sebagai obat penurun demam (antipiretik), akan tetapi jika diminum dalam dosis berlebihan dalam jangka waktu yang lama dapat mengakibatkan kerusakan pada hati. Kedua, obat yang digunakan harus tepat, sehingga dapat mengatasi masalah dan menyembuhkan penyakit yang diderita pasien. Penyakit yang disebabkan karena infeksi virus (misalnya : flu, cacar, dll) seharusnya diobati dengan antivirus bukan dengan obat antibiotic, oleh karena itu tidak tepat jika pasien terkena infeksi karena virus diobati dengan antibiotic, karena akan menimbulkan efek resistensi (bakteri kebal terhadap obat). Ketiga, obat harus digunakan pada pasien yang tepat, jangan sampai pasien yang sebenarnya tidak memerlukan obat justru diberi obat. Keempat, kegunaan obat harus tepat. CTM seharusnya digunakan untuk obat anti alergi, tetapi sebagian besar masyarakat menggunakannya untuk obat tidur, hal tersebut tidak tepat. Kelima adalah waspada terhadap efek samping obat. Hampir setiap obat memiliki efek samping, dari yang ringan sampai efek yang membahayakan dan mengancam jiwa.
Untuk dapat menimbulkan efek, obat harus berada pada tempat kerjanya dalam kadar yang cukup. Untuk mencapai tempat aksinya, obat harus mengalami proses perpindahan dari tempat pemberian ke dalam aliran darah, untuk selanjutnya diangkut (ikut aliran darah) sampai ke tempat aksinya. Proses masuknya obat dari tempat pemberian ke dalam aliran darah disebut absorpsi. Untuk dapat masuk ke dalam aliran darah obat harus dapat menembus sawar (barrier) yang memisahkan obat di tempat obat berada (tempat pemberian) dengan tempat kerja obat. Kecepatan absorpsi obat dipengaruhi oleh banyak factor, diantaranya oleh cara pemberiannya. Obat yang diberikan melalui saluran cerna, absorpsinya lebih lambat dari pada yang diberikan secara injeksi (suntikan). Faktor lain yang berpengaruh pada kecepatan absorpsi antara lain kelarutan obat dalam lemak dan air.
Dalam aliran darah, obat dapat berada dalam keadaan bebas atau dapat pula terikat oleh protein plasma. Kedua bentuk itu kemudian ikut aliran darah (distribusi) ke seluruh bagian tubuh. Hanya obat bebas saja (tidak terikat oleh protein plasma) yang dapat mencapai tempat kerjanya, kemudian menimbulkan efek, baik sebagai efek yang diharapkan (efek terapi) maupun efek yang tidak diharapkan (efek samping dan efek toksik). Obat yang dapat mencapai organ tertentu (misalnya hepar) akan mengalami perubahan secara biologik (biotransformasi) menjadi senyawa lain agar lebih mudah dikeluarkan (ekskresi) dari dalam tubuh. Hampir semua obat dikeluarkan dari dalam tubuh (ekskresi) bersama urin oleh ginjal.
Kecepatan timbulnya efek tergantung pada kecepatan suatu obat sampai di tempat kerjanya. Ini ditentukan oleh kecepatan absorpsi yang dipengaruhi oleh banyak faktor, antara lain cara pemberian obat. Pada garis besarnya cara pemberian obat dapat dilakukan melalui saluran cerna (enteral), atau cara lain yang tidak melalui saluran cerna (parenteral). Pemberian secara enteral dapat secara oral (diminum), sublingual (ditaruh di bawah lidah), buccal (ditaruh di antara gusi dan mukosa pipi) atau perektal (dimasukkan ke dalam rektum melalui dubur). Pemberian secara parenteral dapat dilakukan melalui saluran nafas (inhalasi), dimasukkan vagina (pervaginam) atau disuntikkan secara intravena (iv) yaitu disuntikkan langsung ke dalam pembuluh darah vena, intramuskuler (im) yaitu disuntikkan masuk ke dalam otot daging, intraperitoneal (ip) yaitu disuntikkan langsung ke dalam rongga perut, subkutan (sc) yaitu disuntikkan di bawah kulit ke dalam alveola, intrakutan (ic) yaitu disuntikkan sedikit dalam kulit untuk tujuan diagnosa, intracardial yaitu disuntikkan langsung ke dalam jantung, intraarterial yaitu disuntikkan langsung ke dalam pembuluh darah vena yaitu disuntikkan langsung ke dalam pembuluh darah arteri, dan sebagainya. Obat juga dapat diberikan secara topikal (langsung ditaruh pada tempat atau daerah yang diobati).
Bentuk sediaan obat dapat merupakan :
• sediaan padat (pulvis/serbuk adalah campuran kering bahan obat atau zat kimia yang dihaluskan untuk pemakaian oral atau untuk pemakaian luar; tablet adalah sediaan padat yang mengandung bahan obat dengan atau tanpa bahan pengisi; pil adalah bentuk sediaan padat berupa masa bulat mengandung satu atau lebih bahan obat dan dimaksudkn untuk pemakaian secara oral; kapsul adalah sediaan padat yang terdiri dari obat dan cangkang keras atau lunak yang dapat melarut; implan atau pelet adalah sediaan dengan masa padat steril berukuran kecil berisi obat dengan kemurnian tinggi dibuat dengan cara pengempaan atau pencetakan; suppositoria adalah suatu bentuk sediaan padat yang pemakaiannya dengan cara memasukkan melalui lubang atau celah pada tubuh biasanya melaui rektum, vagina, atau saluran urin)
• sediaan setengah padat (cream adalah bentuk sediaan setengah padat yang mengandung satu atau lebih bahan obat terlarut atau terdispersi dalam bahan dasar yang sesuai; pasta adalah sediaan semi padat yang mengandung satu atau lebih bahan obat yang ditujukan untuk pemakaian topical; gel merupakan system semi padat terdiri dari suspensi yang dibuat dari partikel anorganik yang kecil atau molekul organic yang besar terpenetrasi oleh suatu cairan; salep adalah sediaan setengah padat ditujukan untuk pemakaian topical pada kulit atau selaput lender)
• sediaan cair (sirup adalah suatu larutan obat dalam larutan gula yang jenuh, biasanya diberi esen; suspensi adalah suatu campuran obat berupa zat padat terbagi halus yang terdispersi di dalam medium cair, harus dikocok sebelum digunakan; emulsi adalah suatu campuran dua zat cair yang tidak mau campur; tingtura adalah larutan yang mengandung etanol atau hidroalkohol dibuat dari bahan tumbuhan atau senyawa kimia; eleksir adalah suatu larutan alkoholis dan diberi pemanis mengandung obat dan diberi bahan pembau/aroma; magma : sediaan yang mengandung obat padat terbagi halus terdispersi dalam cairan; lotion adalah preparat cair yang dimaksudkan untuk pemakaian luar pada kulit).
Bentuk sediaan dimaksudkan untuk memudahkan pemberian, memberi rasa atau aroma yang enak, mencegah kerusakan bahan aktif oleh enzim dalam saluran cerna, mempertahankan kadarnya dalam darah dan mempercepat onsetnya (waktu antara obat diberikan sampai menimbulkan efek). Sediaan obat dalam bentuk tablet sublingual misalmya, cepat masuk dalam aliran darah sistemik juga dapat terhindar dari kerusakan bahan aktif akibat getah saluran cerna.
Pada umumnya obat mempunyai lebih dari satu aksi atau efek. Efek obat yang diharapkan untuk menyembuhkan penyakit disebut dengan efek terapi. Misalnya ketika demam, kemudian diberikan parasetamol sehingga demamnya hilang, maka efek terapi dari parasetamol adalah menurunkan demam (antipiretik). Selain itu ada efek suatu obat yang tidak termasuk dalam kegunaan terapi, efek tersebut dinamakan efek samping obat. Contohnya CTM efek samping yang ada yaitu menindurkan. Efek samping morfin adalah depresi pernafasan dan susah BAB (konstipasi). Efek yang lain adalah toksisitas (efek racun), yaitu aksi tambahan yang derajatnya lebih tinggi dibanding efek samping dan merupakan efek yang tidak diinginkan. Efek toksik umumnya timbul pada pemakaian obat dengan dosis yang berlebihan (di atas dosis normal). Contohnya, mual (nausea) dianggap efek samping tetapi depresi sumsum tulang belakang disebut toksisitas. Ada pula efek teratogen, yaitu efek dari obat pada dosis terapetik untuk ibu, mengakibatkan cacat pada janin. Misalnya focomelia (kaki dan tangan seperti kepunyaan singa laut).
Beberapa obat juga memiliki efek yang mungkin timbul pada pengulangan penggunaan obat atau perpanjangan penggunaan obat. Efek tersebut yaitu : reaksi hipersensitif, merupakan suatu reaksi alergi yaitu seatu respon abnormal terhadap obat atau zat dimana pasien telah kontak atau menggunakan obat tersebut yang mengakibatkan timbulnya antibodi. Reaksi kumulatif, adalah suatu fenomena pengumpulan obat dalam badan sebagai hasil pengulangan penggunaan obat, dimana obat diekskresikan lebih lambat dari pada absorbsinya. Toleransi, ialah suatu fenomena berkurangnya besar respon terhadap dosis yang sama dari obat, dosis harus diperbesar untuk mendapatkan respon yang sama. Resistensi, yaitu kemampuan mikroorganisme untuk menahan efek obat yang mematikan terhadap sebagian besar anggota spesiesnya (bakteri menjadi kebal terhadap obat yang diberikan). Habituasi, adalah gejala ketergantungan psikologik terhadap suatu obat. Adiksi, adalah gejala ketergantungan psikologik dan fisik terhadap suatu obat.
Berdasarkan undang-undang dan peraturan yang berlaku di Indonesia, obat dapat digolongkan menjadi : obat bebas, obat bebas terbatas, obat keras, obat narkotik, dan obat psikotropik. Penggolongan obat dilakukan dalam rangka pengamanan dan peningkatan pengawasan obat yang beredar di masyarakat. Obat bebas adalah obat yang dapat dibeli tanpa resep dokter. Obat bebas dapat diperoleh di warung, toko obat berizin atau apotek. Dalam kemasan obat disertai brosur yang berisi nama obat, nama dan isi zat berkhasiat, indikasi, dosis dan aturan pakai, nomor batch, nomor registrasi, tanggal kadaluawarsa, nama dan alamat pabrik serta cara penyimpanannya. Obat bebas diberi tanda bulatan berwarna hijau dengan garis tepi berwarna hitam, diameter minimal 1 cm. Contoh obat bebas : tablet vitamin C 50 mg, 100 mg, 250 mg; tablet vitamin B1 50 mg, 25 mg, 100 mg; tablet vitamin B complex; tablet multivitamin; salep 24; boorwater. Obat bebas terbatas yaitu obat yang digunakan untuk mengobati penyakit ringan yang dapat dikenali oleh penderita sendiri. Obat bebas terbatas (obat daftar W) termasuk obat keras dengan batasan jumlah dan kadar isi berkhasiat dan harus ada tanda peringatan. Obat bebas terbatas ini dapat diperoleh di warung, toko obat berijin dan apotek. Obat bebas terbatas diberi tanda bulatan berwarna biru dengan garis tepi berwarna hitam, diameter minimal 1 cm. Obat bebas terbatas mempunyai penandaan berupa tanda peringatan yang terdapat pada wadah, bungkus luar kemasan obat. Tanda peringatan tersebut berupa :
• P1 : awas obat keras! bacalah aturan memakainya!
• P2 : awas obat keras! hanya untuk kumur, jangan ditelan!
• P3 : awas obat keras! hanya untuk bagian luar dari badan!
• P4 : awas obat keras! Hanya untuk dibakar!
• P5 : awas obat keras! Tidak boleh ditelan!
• P6 : awas obat keras! Obat wasir, jangan ditelan!
Contoh obat-obat bebas terbatas : anusol (obat wasir), antimo (anti muntah dalam perjalanan), dulcolax (obat pencahar), tablet vitamin K 1,5 mg (anti perdarahan), dan lain-lain. Obat keras (obat daftar G) adalah obat yang hanya boleh diserahkan dengan resep dokter. Semua obat yang digunakan dengan cara disuntikkan juga termasuk obat keras. Obat keras ini hanya dapat diperoleh di apotek dan harus dengan resep dokter atau pengawasan apoteker di apotek. Obat keras diberi tanda bulatan berwarna merah, garis tepi berwarna hitam, degan huruf K berwarna hitam di tengah, diameter minimal 1 cm. Contoh obat yang termasuk obat keras : semua obat injeksi (obat suntik), obat antibiotika, diazepam (obat penenang), yonhimbin (aprodisiaka), hydantoin (obat anti epilepsi), nitroglycerin (obat jantung).
Narkotika (obat daftar O) adalah zat atau obat yang berasal dari tanaman atau bukan tanaman yang dapat menyebabkan penurunan atau perubahan kesadaran, hilangnya rasa, mengurangi sampai menghilangkan rasa nyeri dan dapat menimbulkan ketergantungan. Obat narkotik hanya dapat diperoleh di apotek dan harus dengan resep dokter. Undang-undang narkotika membagi obat-obat golongan narkotik menjadi riga golongan, yaitu :
• Narkotika golongan I : hanya digunakan untuk tujuan pengembangan ilmu pengetahuan dan tidak dalam terapi, karena mempunyai potensi sangat tinggi mengakibatkan ketergantungan. Termasuk golongan ini antara lain : cocain, marihuana, heroin, tetrahydrocannabitol.
• Narkotika golongan II : dapat digunakan dalam terapi selain untuk tujuan ilmu pengetahuan, tetapi juga mempunyai potensi tinggi mengakibatkan ketergantungan. Termasuk golongan ini antara lain : morphine, dihydromorphine, hydrocodone.
• Nerkotika golongan III : banyak digunakan dalam terapiselain untuk tujuan ilmu pengetahuan, mempunyai potensi ringan mengakibatkan ketergantungan. Termasuk dalam golongan ini antara lain : codeine, dihydrocodeine, ethylmorphine.
Psikotropika adalah zat atau obat baik alamiah maupun sintesis bukan narkotika yang berpengaruh pada susunan saraf pusat yang menyebabkan perubahan khas pada aktivitas mental dan perilaku. Berdasarkan undang-undang tentang psikotropika, obat-obat jenis psikotropika digolongkan menjadi empat golongan, yaitu :
• Psikotropika golongan I : hanya digunakan untuk tujuan ilmu pengetahuan, tidak diberikan dengan resep. Termasuk dalam golongan ini antara lain : MDMA (ectasy), psilocybin, psilosin.
• Psikotropika golongan II : boleh diresepkan, tetapi harus disadari bahwa dapat menyebabkan ketergantungan yang besar, apalagi kalau diberikan dalm jangka waktu lama. Termasuk dalam golongan ini antara lain : amphetamine, methaqualone, secobarbital.
• Psikotropika golongan III : boleh diresepkan, tetapi juga dapat memberikan ketergantungan pada penggunaan jangka lama. Termasuk dalam golongan ini antara lain : amobarbital, pentobarbital, glutetimide.
• Psikotropika golongan IV : sering kali diberikan dalam resep, relatif kurang memberikan efek ketergantungan, tetapi tetap harus diwaspadai pada pemberian jangka lama. Termasuk dalam golongan ini antara lain : diazepam, chlordiazepoxide, meprobamate.

Systemic steroids

Systemic corticosteroids (glucocorticoids) are synthetic derivatives of the natural steroid, cortisol, which is produced by the adrenal glands. They are called "systemic" steroids if taken by mouth or given by injection as opposed to topical corticosteroids, which are applied directly to the skin. Systemic steroids include prednisone, prednisolone, methylprednisolone, betamethasone, dexamethasone, triamcinolone and hydrocortisone.

Systemic steroids work in the same way as natural cortisol, and are prescribed for a large number of serious diseases. Skin conditions treated with steroids include blistering diseases such as pemphigus and pemphigoid, and severe forms of dermatitis.

What is the role of natural corticosteroids?

Natural cortisol has important effects in the body, including regulation of:

* Protein, carbohydrate, lipid and nucleic acid metabolism
* Inflammation and immune response
* Distribution and excretion of water and solutes
* Secretion of adrenocorticotrophic hormone (ACTH) from the pituitary gland.

What doses of systemic steroids are used?

Systemic steroids vary in strength. The beneficial effects as well as the side effects are proportional to the dose taken. Steroid dose is commonly characterised into:

* Low dose (e.g. <10mg/day>20mg/day of prednisone, sometimes more than 100mg/day).

Treatment for less than one month is considered short term treatment. Treatment continuing for more than 3 months is regarded as long term, and results in the majority of undesirable side effects.

Corticosteroids for a few days or weeks are relatively safe, e.g. for acute dermatitis.

One must always carefully assess the severity of the underlying disorder, the gains that can be expected from corticosteroid therapy, and the risks. Excessive corticosteroid use is one of the causes of Cushing's syndrome.
Skin problems from systemic steroids

The skin is prone to the following adverse effects from prolonged courses or high doses of systemic steroids. These may include:

* Increased risk of skin infections such as bacterial infections (e.g. cellulitis) and fungal infections (e.g. tinea, candida)
* Skin thinning resulting in easy bruising (purpura), skin tearing after minor injury, slow healing, and stretch marks (striae).
* Acne: clusters of small spots on face, chest and upper back.
* Subcutaneous lipoatrophy (loss of fat under the skin surface) from injected steroid that does not go deep enough into the muscle.

Side effects from a short course of systemic steroids

If systemic steroids have been prescribed for one month or less, side effects are rarely serious. However the following problems may arise:

* Sleep disturbance
* Increased appetite
* Weight gain
* Psychological effects, including increased or decreased energy

Rare but more worrisome side effects of a short course of corticosteroids include: mania, psychosis, heart failure, peptic ulceration, diabetes and aseptic necrosis of the hip.

Side effects from a longer course of systemic steroids

Nearly everyone on systemic steroids for more than a month suffers from some adverse effects. These may include any of the following problems, which are not listed in any particular order of importance.

* Reduction of your own cortisol production. During and after steroid treatment, the adrenal gland produces less of its own cortisol, resulting from hypopituitary-pituitary-adrenal (HPA) axis suppression. For up to twelve months after the steroids are stopped, the lack of steroid response to stress such as infection or trauma could result in severe illness.
* Osteoporosis (thinning of the bones) particularly in smokers, postmenopausal women, the elderly, those who are underweight or immobile, and patients with diabetes or lung problems. Osteoporosis may result in fractures of the spine, ribs or hip joint with minimal trauma. These occur after the first year in 10-20% of patients treated with more than 7.5mg prednisone daily. It is estimated that up to 50% of patients using oral corticosteroids will develop bone fractures.
* Reduction in growth in children, which may not catch up when the steroids are discontinued (but it usually does).
* Muscle weakness, especially of the shoulder muscles and thighs.
* Rarely, avascular necrosis of the femoral head (destruction of the hip joint).
* Precipitation or aggravation of diabetes mellitus (high blood sugar).
* Increase in circulating blood fat (triglycerides).
* Redistribution of body fat: moon face, buffalo hump and truncal obesity.
* Salt retention: leg swelling, raised blood pressure, weight increase and heart failure.
* Shakiness and tremor.
* Eye disease, particularly glaucoma (increased intraocular pressure) and posterior subcapsular cataracts.
* Psychological effects including insomnia, mood changes, increased energy, excitement, delirium or depression.
* Headaches and raised intracranial pressure.
* Increased susceptibility to internal infections, especially when high doses are prescribed (e.g. tuberculosis). Avoid oral live polio vaccination. It is safe to have other routine immunisations.
* Peptic ulceration, especially common in those also taking anti-inflammatory medications.
* There are also side effects from reducing the dose; these include tiredness, headaches, muscle and joint aches and depression.

If you have been prescribed systemic steroids, make sure you understand how to take the medicine safely. Regular monitoring during treatment may include:

* Blood pressure
* Body weight
* Blood sugar

Discuss any side effects you may experience with your doctor.
Prevention of osteoporosis

Specific measures to reduce the chance of steroid-induced osteoporosis should be considered for patients that have taken or are expected to take 10 mg or more of prednisone or prednisolone each day for a period of three months or longer.

A DEXA bone scan measures bone density. Bone density gives an indication of the risk of fracture due to bone loss. Arrange to have a scan as you start systemic steroids, and it should be repeated every year or as recommended by your physician.

Preventative treatment includes the following medications:

* Calcium tablets 500- 1000 mg per day
* Vitamin D in various forms including monthly cholecalciferol 50,000 units (1.25 mg)
* Oestrogen i.e. hormone replacement tablets in females that have had early menopause
* Bisphosphonates (alendronate, etidronate); these are prescribed for high risk patients.

Treatment is most effective when started at the same time as the steroids, as most bone loss occurs within the first few months. This is most important for people taking more than 7.5mg of prednisone (or the equivalent dose of another oral corticosteroid) for three months or more.

If you smoke, stop. Consume minimal alcohol. Take regular weight bearing exercise e.g. walking for 30 to 60 minutes each day.

Reducing the dose of systemic steroids

Do not suddenly stop systemic steroids; your doctor will explain how to gradually come off them (particularly important if you have been on them for more than six weeks). For example:

* No tapering is necessary if the course of steroids has been for less than one week.
* After taking a dose of 30 mg or more per day for 3-4 weeks, reduce the dose by 10 mg or less per day, taking days to weeks to stop altogether.
* A much slower reduction in dose may be required if the medication has been taken for several months.

Vicks VapoRub Linked to Infant Breathing ProblemsTUESDAY, Jan. 13 (HealthDay News) — The popular cold remedy Vicks VapoRub may cause airway inflammation that can restrict breathing in infants and toddlers, a new study says.


Doctors at Wake Forest University started their study after treating an 18-month-old girl who had developed severe respiratory distress after the salve had been put directly under her nose to relieve cold symptoms.
“The company is really clear that you don’t put it in the nose, and you never use it in kids under 2,” said lead researcher Dr. Bruce K. Rubin, professor and vice chair for research at Wake Forest’s Department of Pediatrics. “Sure enough, when we stopped all the medicine, the child got much better very quickly.”

Rubin’s experience prompted him to see if there had been other similar cases. “We encountered a few others that appeared to develop problems after using Vicks VapoRub. Parents never volunteered it, because they always thought it is just something you buy over-the-counter, and it’s not a real medicine, because you just rub it on, after all,” he said.

Rubin said Vicks VapoRub can make some adults feel better without really making them better. “For kids, because it can induce some inflammation, even a little bit, that little bit might be enough to tip over a child to having problems,” he said.

The findings were published in the January issue of the journal Chest.
To test whether Vicks VapoRub could cause respiratory distress, the researchers conducted experiments with ferrets. The animals were chosen because they have airways similar to human airways, Rubin said.
The researchers found that Vicks VapoRub increased mucus production by up to 59 percent; the ability to clear mucus was reduced by 36 percent.
David Bernens, a spokesman for Proctor & Gamble, the makers of Vicks VapoRub, doesn’t think one incident involving one child means that the product is unsafe.

“The product is safe and effective when used as directed,” he said. “To say it was the Vicks VapoRub that caused the respiratory distress — I’m not sure we have made that link yet.”

Dr. James A. L. Mathers Jr., president of the American College of Chest Physicians, said in an association news release: “Parents should consult with a physician before administering any over-the-counter medicine to infants and young children. Furthermore, the American College of Chest Physicians and several other health-care organizations have concluded that over-the-counter cough and cold medicines can be harmful for infants and young children and are, therefore, not recommended.”

In October, major manufacturers and the U.S. Food and Drug Administration announced that over-the-counter cough and cold medicines should not be used by children younger than 4 years old.

Dr. Daniel Craven, a pediatric pulmonologist at University Hospitals Rainbow Babies and Children’s Hospital in Cleveland, said parents shouldn’t use Vicks VapoRub, because it has no medicinal value and may even be dangerous.

“Previous research has failed to demonstrate any respiratory benefits of VapoRub, and conscientious pediatricians have thus usually tried to dissuade families from spending money on this and similarly ineffective therapies,” Craven said. “Although the findings are someone limited, this study raises the possibility that this product may not just be ineffective, but possibly might have adverse respiratory consequences — particularly if there is an intense exposure — as when it is applied directly under the nostrils.”

FDA Requires Boxed Warning and Risk Mitigation Strategy for Metoclopramide-Containing Drugs

Agency warns against chronic use of these products to treat gastrointestinal disorders

The U.S. Food and Drug Administration announced today that manufacturers of metoclopramide, a drug used to treat gastrointestinal disorders, must add a boxed warning to their drug labels about the risk of its long-term or high-dose use. Chronic use of metoclopramide has been linked to tardive dyskinesia, which may include involuntary and repetitive movements of the body, even after the drugs are no longer taken.

Manufacturers will be required to implement a risk evaluation and mitigation strategy, or REMS, to ensure patients are provided with a medication guide that discusses this risk.

“The FDA wants patients and health care professionals to know about this risk so they can make informed decisions about treatment,” said Janet Woodcock, M.D., director of the FDA’s Center for Drug Evaluation and Research. “The chronic use of metoclopramide therapy should be avoided in all but rare cases where the benefit is believed to outweigh the risk.”

Current product labeling warns of the risk of tardive dyskinesia with chronic metoclopramide treatment. The development of this condition is directly related to the length of time a patient is taking metoclopramide and the number of doses taken. Those at greatest risk include the elderly, especially older women, and people who have been on the drug for a long time.

Tardive dyskinesia is characterized by involuntary, repetitive movements of the extremities, or lip smacking, grimacing, tongue protrusion, rapid eye movements or blinking, puckering and pursing of the lips, or impaired movement of the fingers. These symptoms are rarely reversible and there is no known treatment. However, in some patients, symptoms may lessen or resolve after metoclopramide treatment is stopped.

Metoclopramide works by speeding up the movement of the stomach muscles, thus increasing the rate at which the stomach empties into the intestines. It is used as a short-term treatment of gastroesophageal reflux disease in patients who have not responded to other therapies, and to treat diabetic gastroparesis (slowed emptying of the stomach’s contents into the intestines). It is recommended that treatment not exceed three months.

Metoclopramide is available in a variety of formulations including tablets, syrups and injections. Names of metoclopramide-containing products include Reglan Tablets, Reglan Oral Disintegrating Tablets, Metoclopramide Oral Solution, and Reglan Injection. More than two million Americans use these products.

Recently published analyses suggest that metoclopramide is the most common cause of drug-induced movement disorders. Another analysis of study data by the FDA showed that about 20 percent of patients in that study who used metoclopramide took it for longer than three months. The FDA has also become aware of continued spontaneous reports of tardive dyskinesia in patients who used metoclopramide, the majority of whom had taken the drug for more than three months.

Potential Problems with Insulin Pens in HospitalsIn a recent article, the Institute for Safe Medication Practices (ISMP) highlighted several potential safety problems when hospitals switch from multiple dose vials of insulin to insulin pens.

ISMP points out that there are certain safety advantages in using the pens. For example, the pens may reduce the chance of drug mix-ups, since each pen is pre-labeled with product name and strength, and the patient's name can be on the label as well. But ISMP also notes a number of potential safety problems to watch out for when hospitals switch to pens.
One possible problem is needlestick injuries, which could happen because the pen may make it difficult to see the injection site. Also, some needles do not have needle guards, so a needlestick can occur after the injection.

Another difficulty is that part of the insulin in the pen may not be delivered. This can happen because the buttons on some pens are difficult to push down, making it easy to accidentally lift the needle out of the skin during the injection. There can also be leakage around the injection site if the needle is not left in place for at least six seconds after the injection. In addition, uneven dosing of an insulin suspension can occur if the user fails to tip and roll the pen before the injection, causing the suspension to clump.

Sometimes nurses unfamiliar with a particular type of pen will use it as a multiple-dose vial, withdrawing insulin from the pen cartridge with a sterile needle and a conventional insulin syringe. This is not recommended, because aspirating insulin from the cartridge can leave air pockets. When the cartridge is used again, these air pockets can cause dosing errors or air injection.

Another problem is using the same pen for multiple patients, which is potentially hazardous. Attaching a new sterile needle to the pen before using it on another patient cannot solve the problem. That's because the insulin inside the pen's cartridge can become contaminated with biological material after the first injection, while the original needle is still on the pen.

Finally, the design of some pens can lead to dosing errors. For example, in some cases the digital display of the dose can be mis-read if a person holds the pen upside down, as a left-hander might do. In that case, a dose of 21 units looks like 12.

How can these problems be avoided? ISMP says that the key is to prepare for the transition to pens. That means understanding and anticipating the risks beforehand, doing a failure mode and effects analysis, educating the staff about using the pens properly, and closely monitoring their use for the first few months of the transition. ISMP recommends that written guidelines be developed for each type of pen used in the hospital, with specific instructions on handling the pens safely, injection techniques, and prohibitions on sharing pens or using them as multiple dose vials.
